What is Fat Transfer?

Fat transfer (or “fat grafting”) is a procedure in which fat from an area of the body where it can be spared—usually the thighs, abdomen, or flanks—is removed using liposuction; that fat is then processed and purified, and then is re-injected into the body in area where volume is needed. In this case, the breasts. While breast implants are an excellent foundation for augmentation, fat transfer allows for a more refined appearance.

When used alone to enhance the breasts, fat transfer usually does a better job of creating contour or fullness with more subtle increases in actual size of the breasts.  This may be more desirable for someone who does not really want significantly larger breasts but would like more fullness to fill out bras, swimsuits, or clothing better—all without the need for breast implants.

How Fat Transfer Enhances Breast Implants 

Composite or hybrid breast augmentation—the combination of breast implants and fat transfer— is often an excellent option for an enhanced, natural-looking appearance. Breast implants are fully customizable to a patient’s personal aesthetic goals, body type, and lifestyle. They are much more predictable than fat transfer, especially when it comes to actually enlarging the breasts beyond a cup size, and therefore they create an ideal structure. Fat transfer adds a layer of cushion around the implant, which conceals the edges of the implants, slightly increases the cup size, and balances contour deficits or symmetry. The result is a softer, smoother look for the breasts.

Yes. If you would prefer to avoid the use of implants altogether and achieve a more modest increase in the size of your breasts, fat transfer breast augmentation without implants is an option. Dr. G can discuss this with you during a consultation.

Where can the fat be taken from?

The fat can be harvested from a few different areas including the abdomen, flanks, and thighs.

How long does the procedure take?

The amount of time will vary, but on average the procedure is between three and five hours. This includes time for harvesting the fat, the purification process, and the trasnfer.

The Fat Transfer Breast Augmentation Procedure

During your consultation with Dr. G, the type of breast augmentation incorporating fat transfer best suited for your goals will be determined. If implants are to be used, the type of implants will also be decided. Placement of the scar and implants will also be discussed during the consultation. Fat transfer alone may be performed under either general or local anesthesia with sedation, while composite or hybrid breast augmentation is performed with the patient under general anesthesia. 

Liposuction will be done first to remove fat from an area of the body where it can be spared. This is usually this is the abdomen, thighs, or flanks. Dr. G will create very small incisions in the area so that a cannula can enter to break up and remove the fat. Once the fat has been broken up and removed, it will be processed and purified to remove any contaminants and will then be placed within a syringe so that it can be injected.

If breast implants are being used, they will typically be placed at this time in the usual way as discussed at your consultation.

Once prepared, the fat is then transferred to the breasts, with Dr. G paying careful attention to how much is needed and in which locations.

Recovery from Fat Transfer Breast Augmentation

You will need someone to drive you home because of the general anesthesia or sedation, and you will also need assistance for the next 24 hours. For one week, you’ll wear a protective surgical bra, and then after that you may begin wearing a soft bra without an underwire. You will also wear a compressive garment over the liposuction donor site for the fat. Do not shower for 24 hours, and be sure to take short, regular walks around your home each day to help with circulation. Most of the bruising and swelling should be resolved in two weeks. You should be able to resume sedentary work in one week, and Dr. G will inform you on when you can continue more intense exercise (usually it’s about six weeks). You should be fully healed in two months.
